Information Referral

We can help you find mental health services or supports in the London-Middlesex region.  Since 1950, we have been a resource to consumers,  family members and professionals.  Are you looking for a self-help group?  Counsellor?  Psychiatrist?  Housing? Group therapy?  Income support?  Or, do you simply have a question about mental health in general or mental health services?   Give us a call at 519-434-9191 (Monday to Friday, 8:30 am to 4:00 pm).

Intake

Our Intake Team can help you evaluate your present supports and complete an intake in order to connect you to one + one community support.   To make a referral, please CLICK HERE to obtain a copy of a referral form.    You may fax the form to us at 519-438-1167.

Assessment Services

After the Intake process has been completed, our Assessment Team will facilitate timely and rapid access to ongoing mental health supports, services and treatment as deemed appropriate through consultation, recommendation, consumer feedback and consumer request.   The Assessment Team's main objective is to provide direct access to community support through one point of entry.

Crisis Response Access Coordination:  Serving the Needs of the Consumer at the Time of Crisis

Crisis Supports are offered and referred internally when a crisis need has been identified through either our Information Referral Team, Assessment Team or our community partners.    A crisis staff assists individuals with one or two crisis needs and facilitates a connection to longer term supports.

CMHA London-Middlesex has two Crisis Response Access Coordinators that work directly with Women's Community House.    These services are available to any woman at Women's Community House that is experiencing a mental health crisis.
